Insurance Participation

We participate with most major insurance companies, HMO’s, PPO’s, POS’s and Medicare.

Call the office for a complete listing. Please take note of information below insurance listing.

We accept:

  • Aetna (Choice PPO, Choice POS II, Innovation, Foreign Service Benefit Plan, Advantage Medicare Supplement, Coventry. HMO might require referral.)
  • BCBS (Anthem, CareFirst, Federal Employee, Open Access, GEHA, Illinois, Healthy Blue, Premera, Advantage, Medicare Advantage) 
  • United Health Care (Compass Rose, Choice Plus. HMO might require referral.)
  • PHCS Multiplan 
  • Medicare (AARP Medicare Supplement and Mutual of Omaha)
  • Tri-Care (referral needed for most plans)  
  • Cigna: Open Access Plus, ChoiceFund OA Plus with CareLink, Choice Fund PPO, Teamsters Local, Medicare Advantage. HMO might require referral.)
  • Any plans that are HMO, please contact insurance to verify and if a referral is required or not.

We do NOT accept:

  • Aetna (Better Health)
  • BCBS (Anthem Health Keepers)
  • United Health Care (MediPlus, Compass, Commonwealth Coordinated Care, Dual Complete, InTotal Health)
  • Cigna (Connect and EPO)
  • All Medicaid plans (even if combined with other plans; ie Aetna Medicaid)
  • Magellan
  • Kaiser

We cannot accept patients whose secondary insurance is not in network with the practice. Patients who schedule a telehealth appointment are required to email or fax their insurance card and intake forms. If you are self-pay, we will NOT provide a claim. $200 initial visit, $100 follow-up.

This gentle physical exfoliant can be done 2-3 times a week and can be found at Ulta or Sephora.

Dermalogica Microfoliant

This is vitamin C ‘skinceuticals’ dupe; apply this once to twice a day under your makeup/moisturizer. Don’t forget to apply this to your hands, neck and chest as well! Helps to brighten the skin and fight sun damage caused by free radicals

Vitamin C Serum

This is a mineral based sunscreen that is powdered so it makes reapplication easier. Its also translucent so you do not have to shade match.

Powdered Sunscreen

This is my favorite night time moisturizer. Its very thick so a little goes a long way.
